All forms should be provided to each individual reporter. To determine who the court reporter was, you must review the docket entry for the proceeding. When you pick the reporter at the top of the Transcript Order Form, the reporter's email will appear on the bottom of the form so you will know where to send the form. You will be contacted about payment arrangements, which MUST BE MADE before delivery deadlines will be honored. If no docket entry has been filed for your proceeding, contact the presiding judge's courtroom deputy or email the court reporter supervisor.
